TULONG PANG-EDUKASYON Program (Educational Assistance) for 2nd Semester AY 2024-2025

Deadline of Application and Submission of Documents is March 18, 2025 (5:00PM), Tuesday

This educational assistance program of Hon. Mohamad “Tong” P. Paglas, Sr will cater to FOURTH YEAR OR GRADUATING STUDENTS whose place of residence is Maguindanao del Sur. The student-beneficiaries shall receive financial assistance if they meet the following QUALIFICATIONS:

  • Currently enrolled in a 4- or 5-year degree college program in ANY school, college, or university accredited by CHED (should be FOURTH YEAR OR GRADUATING STUDENT).
  • Registered voter or resident of Maguindanao del Sur.
  • Did not receive any educational assistance from DSWD as of 2nd Semester of AY 2024-2025.

The student is expected to provide the following DOCUMENTARY REQUIREMENTS:

  1. Application Form for the Hon. Mohamad “Tong” P. Paglas Sr Tulong Pang-Edukasyon Program (2nd Semester, AY 2024-2025) – 2 original
  2. Proof of Enrollment – any of the following:
    a. School ID with validation or stamp showing enrollment in current semester – 2 photocopies
    b. Statement of Account (SOA) or Certificate of Enrollment/ Registration (COE/COR) – 2 original
  3. Proof of Residency in Maguindanao del Sur – any of the following:
    a. Voter’s/COMELEC ID or Certificate of Voter’s Registration – 2 photocopies
    b. Photocopy of any valid ID such as Driver’s License, National ID/ ePhilID, or Passport – 2 photocopies
  4. Accomplished DSWD Form – General Intake Sheet – 2 original
  5. Accomplished DSWD Form – Certificate of Eligibility – 2 original
  6. Certificate of Indigency – Purpose: Financial Assistance – 2 original

So, if you think you meet the qualifications and would like to apply to Hon. Mohamad “Tong” P. Paglas’ Educational Assistance Program, make sure to strictly follow the PROCEDURE below:

STEP #1 FILL UP THE ONLINE APPLICATION FORM.
(ONLINE APPLICATION FORM). You should receive a copy of your submission via e-mail.
STEP #2 ACCOMPLISH ALL DOCUMENTARY REQUIREMENTS.
See above for the list of 6 documents to be submitted.
STEP #3 SUBMIT DOCUMENTS FOR INITIAL CHECKING.
Submission will be at the Office of Cong. Paglas. Address: Shield Commercial Center, Poblacion, Datu Paglas, Maguindanao del Sur, from Monday to Friday, 9 to 5PM only.
